Individual Therapy: $135.00

 

  • Couples/ Family Therapy: $220.00​ | Deposit Fee: $100 

   ​

  • No Show Appointments: $50
     

  • Late Cancellations (Less than 24 Hours): $25

  • Request of Patient Clinical Notes: $30

  • FMLA and Disability Paperwork (Processed within 7-14 business days): $75

  • Expedited FMLA and Disability Paperwork (Processed within 3-7 business days): $100

Please note: Completion of FMLA paperwork requires a minimum of three (3) full sessions with your clinician prior to submission.

Requests for FMLA extensions or additional supporting documentation require at least two (2) additional sessions to be completed before submission.

  • Request of Form/Letter Completion (Processed within 3-7 business days): $40

  • Expedited Form/Letter Completion: $65

Insurance Responsibility Notice: As a valued client of our mental health services, please note that it is your responsibility to notify our office staff promptly of any changes to your insurance provider, plan, or policy. Failure to do so may result in out-of-pocket expenses or delayed treatment. By staying informed and proactive about your insurance coverage, we can work together to ensure seamless care and support.
