Financial and Appointment Policy

Financial Responsibility

Client accounts must remain current. If a balance reaches $50 or more, services may be paused until payment is made in full or a payment plan is established.

Payment is required at the time of service unless otherwise arranged in writing.

Clients are responsible for all costs associated with services, including copays, coinsurance, deductibles, and any out-of-pocket fees.

If a payment plan is approved, adherence is required. Failure to comply with an agreed payment plan may result in suspension or termination of services.

 

Payment Policy

Payment is required prior to the start of each session.

If a payment does not process successfully, a payment link will be sent and must be completed before or at the time of check-in.

Services may be delayed or rescheduled until payment is received.

 

Appointment Policy

Appointments are scheduled for 30–60 minutes, depending on the service provided.

Clients are expected to arrive on time. Late arrivals will still end at the scheduled appointment time, and the full session fee will apply.

  • Late cancellation (less than 24 hours’ notice): $25 fee

  • No-show (failure to attend without notice): $50 fee

  • Missed appointments must be paid in full.

  • For Medicaid/Medicare clients, after one no-show without notice, future scheduling may be restricted in accordance with program requirements.

Fees

  • Individual Therapy: $135

  • Couples/Group Therapy: $220

  • Late Cancellation: $25

  • No-Show: $50

  • FMLA paperwork: $75 (standard processing: 7–14 business days)

  • Expedited FMLA: $100 (3–7 business days)

  • Forms/Letters: $40 (3–5 business days)

  • Expedited Forms/Letters: $65 (3–7 business days)

Insurance Note

No reimbursements will be issued unless a duplicate payment is received directly from your insurance provider.

Clients are responsible for verifying coverage and understanding their insurance benefits prior to services.

 

Agreement

By engaging in services, the client acknowledges that they have read, understood, and agree to the terms outlined in this Financial & Appointment Policy.
